Who We Are

Health-Bay by Baycrest is a web platform geared towards seniors residing in Toronto Seniors Housing with the aim of providing them with access to health information and education resources; self-assessments to learn about their own well-being; access to useful resources and the ability to request support from the Neighbourhood Care Team members in their building.  

The platform can be accessed through tenants’ own devices such as smartphones, iPads, tablets or computers and is also available via a physical kiosk in a central location at each building to enable access to those who do not own a device or do not have access to internet in their unit.  

Baycrest  

Baycrest is an academic health sciences centre providing a continuum of care for older adults, including independent living, assisted living, long-term care and a post-acute hospital, all within one campus. The organization is a global leader in geriatric residential living, healthcare, research, innovation and education, with a special focus on brain health and aging. 

What is a Neighbourhood Care Team 

Baycrest operates the Health-Bay website in collaboration with the Neighbourhood Care Team in each TSHC building in order to allow tenants to engage with their team to request their support and be informed of health and recreational activities offered in their building.  

A Neighbourhood Care Team (NCT), is a model of care, which aims to make it easier for you to access health and social services by bringing services closer to your home or to your building. The team is made up of local health and community care providers who work together to coordinate your care and make it easy for you to access services and supports. Anyone in the NCT can help you connect to various services. Some of our NCT members may be working within your neighbourhood or building, and you can reach out to them directly to let them know you need support or services from your team. In your building, this may include clinicians such as social workers or nurses who regularly visit your building to provide support and help you connect to other services as needed.
